To Make an Appointment Please Complete our

New Client Intake Form

If you begin our Intake Form and are unable to complete it at that time, please select the “Finish Later” option at the top right corner of the form to save your progress and return to the form when it is convenient for you.

All prospective patients must complete our Intake Form. Once completed, you will be contacted within 24 to 48 hours to schedule your first appointment.

If you do not have computer access or if you require other accommodations, you may call (334) 262-7787 and Dial “1” to reach our intake coordinator who will assist in gathering your information.

If you would like to see a specific counselor from our staff, please request on the intake form.

Once we receive all of your information, our Clinical Director will review and assign a counselor with whom to schedule your appointment, taking into consideration your need and your health insurance status.

Eligible individuals without insurance coverage are seen regardless of ability to pay and are assigned a sliding scale fee based on documented gross income.  Gross income can be verified by one or more of the following: pay stub, bank statement, income tax return, copy of alimony or child support check, statement of Social Security Benefits or unemployment check. 

Forms for Your First Visit

All new clients should read the “Consent for Services Form” and the “Notice of Privacy Practices” prior to their first visit.

After you’ve been scheduled for your first appointment you will be given access to your client portal. Different clinical questionnaires will be assigned to your portal. You MUST complete these before your first appointment. If these are not completed you may not be able to keep your first appointment. If you require assistance with these questionnaires or require other accommodations please call our office at (334) 262-7787 and dial “1” to speak with our intake coordinator.
